Requesting Your Medical Records
You can quickly and easily access your health records through the PCI Patient Portal or by using PCI's healow app.
If you would like to request records, including imaging and lab results, beyond what is provided in your healthcare summary, sent to either yourself or a physician, please follow these instructions for requesting your medical records:
In-Person
To request your medical records in person, please come to PCI Medical Pavilion anytime between 8 a.m. – 5 p.m., Monday – Friday. You can request your records from the reception desk at the clinic where you were seen.
A medical records specialist is available Monday – Friday, 8 a.m. – 2 p.m., on the 1st floor of PCI Medical Pavilion 1 if you have questions or special requests.
Please bring photo identification with you. This is required to request medical records in person.
By Mail
Physicians’ Clinic of Iowa has retained a professional service, Healthmark Group, to handle the duplication and transfer of medical records.
In order to expedite all requests for patient information:
- Download the Medical Record Release of Information Authorization form.
- Sign, date, and completely fill out the Medical Record Release of Information Authorization form. Include your phone number and complete address on your request in the event of any questions regarding the release of your records.
- Submit your signed and COMPLETED Medical Record Release of Information Authorization to him@pcofiowa.com or fax it to 877-671-3861.
There may be a fee for the transfer of your information.
Records will be delivered via electronic delivery unless otherwise indicated on the Medical Record Release of Information Authorization.
Paper copies may have an additional fee for delivery.
